About The Position

Chestnut Health Systems is seeking a Clinician III (Housing) to provide behavioral health services to individuals in supportive housing programs. This role focuses on delivering therapy, coordinating care, and supporting clients in maintaining stable housing through recovery-oriented, trauma-informed services. The clinician works closely with housing teams, substance use programs, and community partners to ensure comprehensive, client-centered care. We provide fully integrated care for all our patients by combining behavioral health care services with our community-based primary care health centers. Chestnut also conducts research and training throughout the U.S.A. This full-time position will work Monday through Friday from 7:00am to 3:30pm and will be based at our Granite City, IL housing site. The anticipated starting pay for new hires for this position is between $44,400 - $50,575 annual salary. T here are several factors taken into consideration in determining base pay, including but not limited to: job-related qualifications, skills, education, experience, local market conditions, and internal equity.

Requirements

Must have a master’s degree in counseling, psychology, social work or related field with at least 1,000 hours of supervised mental health experience.
Requires basic computer skills and experience to include MS Word, MS Excel, Outlook, Adobe Acrobat and an electronic medical record (EMR).

Responsibilities

Complete timely and accurate documentation, including assessments, treatment/recovery plans, progress notes, and required paperwork in compliance with agency and funder standards
Provide individual, group, and family therapy, along with community support and other clinical services based on client-centered treatment plans
Maintain productivity expectations and complete service authorizations and re-authorizations as required
Screen and assess new housing referrals when assigned
Deliver services in community and home-based settings to support clients in their natural environments
Use evidence-based practices to support recovery, skill-building, and overall wellness
Promote trauma-informed, recovery-oriented care that emphasizes hope, respect, empowerment, and stability in housing
Connect clients to substance use treatment, housing resources, and other community supports through referrals and advocacy
Collaborate with internal teams, including housing and substance use programs, to coordinate care and support positive outcomes
Participate in meetings, staffings, supervision, trainings, and program development activities
Take initiative and provide leadership within the team with minimal supervision
Support team members and provide backup coverage as needed
Maintain strong professional relationships with clients, colleagues, and community partners
Maintain confidentiality and protect all client and organizational information
