About The Position

The Housing Case Manager (HCM) is responsible for case management and supportive care coordination for assigned clients and their households.  The STARS 6 Young Adult family program is a scattered site supportive housing program for young families. The Housing Case Manager works closely with their clients in the clients’ homes, in the community, and the office, helping to ensure housing stability by promoting independent living skills, financial management, and health and wellness.

Requirements

  • Must possess a BSW or related degree with adherence to ethical standards of the social work profession.
  • MSW is preferred
  • Experience with and proven commitment to working with young adults with low-income and marginalized communities, as well as experience working with young children
  • Good communication skills, both verbally and written, including the ability to communicate with all levels of management. 
  • Must be proficient in Microsoft Outlook and Office (Word, Excel, and PowerPoint). 
  • Understanding harm-reduction practices. 
  • Cultural competency, especially around the LGBTQI+ community
  • Dependable and flexible
  • Able to maintain a high level of client confidentiality.
  • Excellent internal and external customer service skills and strong interpersonal skills.
  • Good communication skills both verbally and written, including the ability to communicate with all levels of management.
  • Must be proficient in Microsoft Outlook, Word, and Excel.
  • Ability to work under pressure.
  • Ability to work with colleagues to solve problems.

Responsibilities

  • Effectively manage assigned caseload, i.e. managing engagement, referrals, concrete needs, and client chart compliance
  • Provide referrals and follow-ups when necessary for primary care, substance use, nutrition, clothing, mental health, financial management, and dental. 
  • Identify and schedule case conferences, using case conference tab, with internal and external providers.
  • Utilizing the vast array of internal services available to clients, look to connect (and coordinate) with other departments including: ADHC, Health Services, OASAS, Health Homes, Behavioral Health, Community Partnerships, Legal, and Job-Training.  
  • Maintain chart compliance, including eICare and TMS (Maven Mental Health System), in line with internal and external guidelines
  • Assist clients in obtaining and maintaining benefits like SNAP, SSI, Medicaid, and rental subsidies
  • Participate in interdisciplinary teams, supporting the resident by coordinating with internal and external providers (i.e. Behavioral health, medical, community partners)
  • Conduct and document service provision in eicare as outline by the contract’s deliverables
  • Help new clients move into their apartments. Assist clients with their transition from homelessness to stable housing. 
  • In collaboration with the client, develop strength based individualized goals. Provide constructive feedback on goal participation during scheduled engagement.  
  • Collaborate with Child Development Specialist to ensure all children are being provided with services as needed
  • Assess and discuss financial management with the client, including following up with the client on any rent obligation, maintaining an active subsidy, consistently making utility payments, and food and nutritional needs.  
  • As needed, accompany the client in the community on scheduled appointments.  
  • Participate in bi-weekly supervision meetings with your supervisor and come prepared.
  • Co-facilitate Parenting Journey groups.
  • Co-facilitate group activities focused on themes relevant to the clients and their children.
  • Travel via mass transit with clients or independently in service of client needs.
  • Provide crisis intervention and mediation as necessary.
  • Provide coverage for other Housing Case Managers when needed.
  • Participate in annual staff training, such as Motivational Interviewing I & II, Smoking Cessation, and Trauma-Informed Care.
  • Assists clients with budgeting, provides counseling regarding rent arrears, delivers ADL (Activities of Daily Living) training, and conducts orientation for new clients on program structure.
  • Complete all required reports and paperwork before the prescribed deadlines. 
  • Demonstrate excellent customer service with residents/clients; staff; vendors; and all other relevant stakeholders. 
  • Enthusiastically participate in advocacy and direct actions to end the twin crises of homelessness and HIV/AIDS
