Requirements
- Previous experience in case management or housing services preferred.
- Knowledge of local resources and programs.
- Effective communication and interpersonal skills.
- Ability to work effectively with individuals from diverse backgrounds.
- Vehicle transportation required, no exceptions.
Responsibilities
- Provide case management services to clients.
- Conduct comprehensive assessments to determine clients' needs.
- Develop individualized plans and goals for clients.
- Assist clients in accessing and securing safe and affordable housing and employment options.
- Facilitate client desires in achieving goals.
- Collaborate with community resources and agencies to support clients' stability.
