About The Position

NAMI San Diego & Imperial Counties (National Alliance on Mental Illness) is a nonprofit organization that provides free education, support services, and advocacy to improve the quality of life of everyone affected by mental illnesses. The Ryan White Program supports individuals living with HIV/AIDS by ensuring they are linked to care, adhere to treatment, and achieve viral suppression. Through a whole-person approach, the program addresses both health care and social service needs, recognizing the complex factors that impact health outcomes. The Housing Case Management component specifically helps participants build self-sufficiency and secure stable, affordable housing-an essential foundation for long-term wellness and engagement in care. The Housing Case Manager plays a vital role in supporting individuals living with HIV/AIDS by promoting self-sufficiency and housing stability. This position helps clients by securing affordable housing, managing finances to achieve personal goals, and identifying support systems that enable long-term housing retention. The Case Manager ensures clients are engaged in HIV outpatient ambulatory health services and supported in achieving viral suppression.

Requirements

  • Reliable and consistent transportation.
  • Able to communicate effectively, verbally and in writing.
  • Proficiency with computer software (Microsoft Office: Outlook, Word, Excel) and web navigation.
  • Demonstrated ability to maintain confidentiality.
  • Reliable and consistent attendance.
  • At least one year of experience providing housing navigation or closely related support.
  • Minimum of one year of experience supporting individuals experiencing homelessness or at risk of homelessness, specifically those living with HIV/AIDS.
  • Minimum of one year of experience supporting individuals experiencing mental health or substance use conditions.
  • Strong familiarity with San Diego County's housing resources, social services, and community culture.
  • Exceptional interpersonal skills, positivity, attention to detail, adaptability, and a passion for working in dynamic environments.
  • Excellent planning, organizing, analyzing, problem-solving, and time management skills.
  • Ability to work independently and collaboratively with others.

Nice To Haves

  • Fluency in Spanish, Vietnamese, Tagalog, Korean, Arabic, Farsi, Chinese, or ASL, preferred.
  • Occasional weekend and evening hours.

Responsibilities

  • Conduct comprehensive assessments to determine clients' housing needs, barriers, and eligibility for housing programs.
  • Develop individualized, client-centered housing and care plans in collaboration with clients, focusing on long-term stability and self-sufficiency.
  • Assist clients in locating, securing, and maintaining safe, affordable housing, including support with housing applications, lease agreements, and landlord communications.
  • Educate clients on tenant rights and responsibilities, budgeting, conflict resolution, and financial management to support housing retention.
  • Monitor client progress monthly, reassess as required by funding contracts, and adjust service plans to meet evolving needs.
  • Provide ongoing case management to ensure housing stability, engagement in HIV outpatient ambulatory health services, and progress toward viral suppression.
  • Coordinate outreach, referrals, and enrollment for eligible participants, ensuring timely access to services.
  • Collaborate with internal teams, healthcare providers, mental health professionals, and community partners to ensure a continuum of care.
  • Build and maintain relationships with community organizations and identify opportunities for new partnerships to enhance service delivery.
  • Advocate for clients with landlords, property managers, and housing authorities to resolve tenancy issues and promote housing stability.
  • Ensure compliance with grant and regulatory requirements, including accurate documentation, reporting, and data entry.
  • Support quality improvement initiatives and track program deliverables to evaluate and enhance program effectiveness.
  • Manage a high-need caseload with professionalism, empathy, and cultural competence.
  • Perform other duties as assigned.
