About The Position

The Housing Case Manager provides clinically appropriate and comprehensive case management services to individuals experiencing homelessness who are referred by the Kansas City, Missouri Health Department. The Housing Case Manager provides services that are strengths based and trauma informed to populations that have experienced homelessness, mental illness, substance abuse, trauma, domestic violence, extreme poverty and are potentially a part of the LGBTQ community. This position is responsible for working directly with medical case managers and participants to assess, plan, implement, coordinate, monitor and evaluate the options and services required to meet the participant’s housing needs.

Requirements

  • A Bachelor’s Degree in Social Services or Human Services related field preferred
  • 3-5 years of experience working with individuals who are living with HIV/AIDS, have experienced severe and persistent mental illness, homelessness, substance abuse, trauma, domestic violence and/or extreme poverty
  • Demonstrated ability to work effectively and collaboratively with a culturally and socioeconomically diverse population of participants, service providers and staff
  • Values diversity and possesses an understanding of multicultural and cultural competencies; understands power, privilege and inequity
  • Knowledge of and the ability to interpret and apply laws, rules, policies and procedures relating to housing assistance programs
  • Adapts to changing business needs, conditions and work responsibilities; responds to change with a positive attitude and a willingness to learn new ways to accomplish work activities and objectives
  • Knowledge and demonstrated ability to provide clinically guided supervision and services related to mental illness, physical disability, substance use, domestic violence and the LGBTQ population.
  • Demonstrates responsible personal and professional conduct; accepts personal responsibility for the quality and timeliness of work
  • Applies effective written and verbal communication techniques to convey clear and timely expectations, listens to people’s ideas and concerns, resolves confrontations, disagreements and complaints in a constructive manner
  • Ability to exercise independent judgment, using analysis, experience and perspective in solving problems or communicating decisions or actions
  • Effective interpersonal skills, with the ability to work collaboratively to garner trust, respect and confidence from others

  • MS Office experience, especially Outlook
  • Must pass a criminal records check and pre-employment drug screening
  • Must have valid driver’s license and current automobile insurance

Responsibilities

  • Models positive and healthy relationship building by treating all participants, staff and volunteers equitably with caring, kindness, respect, safety and dignity
  • Follows all policies, procedures, code of conduct and attendance rules of the organization
  • Maintains strict confidentiality of all information and adheres to the policies in the use of computer technology and all other devices
  • Balances the needs of caseload based upon acuity, length of time in the program, state of housing stability, etc., supporting 5-20 individuals at one time
  • Completes an initial needs assessment and acts upon critical needs appropriately and immediately as well as orients participant to the enrolled program, requirements and expectations
  • Assures that basic nutrition, physical health and hygiene needs are being met
  • Completes safety plan to appropriately support participants in shelter and the community, including home visits and accompaniment to appointments
  • Aids participant in the development of participant-driven housing plan with identified short-term and long-term goals; Empowers participant to become involved in their own planning and goal setting
  • Monitors progress towards goals in regularly scheduled appointments with participant and has regular communication with the medical case manager
  • Provides referrals to appropriate service providers internally and externally to assist with meeting goals
  • Practices fidelity to the Housing First Model, working with participants to achieve greater self-sufficiency and independence over time
  • Assures that specific Health Department program goals are consistently being met and provides appropriate case management to foster transition from homelessness to permanent housing
  • Organizes and maintains participant files in a timely fashion and in accordance with professional practice, including conversations, housing plan, progress towards goals and documentation of all interactions and incidents
  • Timely data input into SCOUT and HMIS database to meet funding requirements and statistical reporting; Completes daily notes, case management logs, and all other program specific requirements in a timely manner
  • Assures participant safety in the shelter and transitional living property through frequent and regular tours of units inspecting for any hazards, repair needs, and independent life skills needs.
  • Reports any maintenance needs and/or hazards immediately to supervisor for immediate attention; communicates with participants to ensure safety and awareness
  • Works in collaboration with program and all other agency staff to facilitate a team environment
  • Attends and positively participates in team meetings and commits to group discussions
  • Attends required meetings at the Health Department on a weekly and monthly basis as scheduled
  • Participates in weekly diversion shift
  • Role models effective team behavior; Substitutes for other staff when need arises
  • Demonstrates effective communication skills in building relationships with all employees and participants
  • Creates positive working relationships with community agencies and service providers to facilitate the access to area resources for participants
