About The Position

The Housing Case Management Specialist is responsible for supporting individuals living with HIV and the Matthew 25 team to ensure that patients are assessed for housing needs and connected to appropriate housing resources. This is accomplished by utilizing Matthew 25’s funds derived from Indiana HUD HOPWA Program. This program is designed to support clients who are at risk of homelessness or are currently experiencing homelessness, with the goal of promoting long-term housing stability and health outcomes.

Requirements

  • Bachelor’s degree in Social Work, Human Services, or related field or relevant professional experience in housing case management.
  • Strong interpersonal skills and the ability to engage with diverse client populations.
  • Experience working in a nonprofit or community-based service setting, or a strong desire to work in a mission-driven environment.
  • Familiarity with housing programs (HOPWA, HUD-funded initiatives), and knowledge of the Housing First model.

  • Flexibility and willingness to travel throughout the assigned service region.
  • Valid Driver’s License with at least three (3) years of driving experience and fewer than two (2) moving violations in the past three (3) years.
  • Ability to pass a drug screen and background check.
  • Must be able to communicate in both written and verbal form
  • Ability to read and interpret documents. Ability to write routine documentation and correspondence.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Intermediate computer skills are essential in use of an electronic medical record.
  • Must be detail oriented, have ability to multi-task and possess great interpersonal skills.

Nice To Haves

  • At least two (2) years of experience in direct service or housing case management preferred.

Responsibilities

  • Provide direct housing case management, including assessments, service planning, and ongoing client support.
  • Promote a Housing First approach rooted in harm reduction and client-centered care.
  • Coordinate access to housing assistance and supportive services, including rental, utility, and relocation support.
  • Build and maintain relationships with landlords, property managers, and housing partners to support stable placements.
  • Ensure accurate documentation and compliance with HOPWA, IHCDA, and HUD program requirements.
  • Collaborate with internal teams and travel within the service area to deliver comprehensive, community-based services.
