The Case Manager is responsible for managing patient care within a multidisciplinary treatment team while coordinating closely with: Therapists and physicians External mental health liaisons Referral sources and care providers Insurance companies Parents and guardians This role ensures accuracy in: Utilization review Care management Discharge planning Contract compliance Electronic health record documentation You will be the central point of information flow for both internal teams and external partners. You will help ensure that no detail is missed, no communication breaks down, and no family is left uninformed. This is a role for someone who is highly organized, self-motivated, reliable, and deeply committed to serving vulnerable populations.

Requirements

  • Bachelor’s degree required
  • Valid driver’s license
  • Proof of auto insurance
  • Minimum of 2 years in case management, utilization review, or wellness coordination
  • At least 1 year working with economically disadvantaged, vulnerable, or at-risk youth and/or adults
  • Intermediate proficiency in Microsoft Word, Excel, and Outlook
  • Strong written and verbal communication skills
  • Excellent time management and organizational abilities

Nice To Haves

  • Preferred: Bachelor’s in social work, education, sociology, psychology, counseling, applied behavioral sciences, or criminal justice

Responsibilities

  • Participate in respectful, inclusive teamwork across all departments
  • Coordinate care and maintain strong working relationships with referral sources, providers, and insurers
  • Ensure accurate data entry and authorization documentation in electronic health records
  • Map and maintain contractual relationships, including single case agreements
  • Communicate placement, progress, and discharge updates to external agencies
  • Engage parents/guardians to coordinate meetings, visitations, and follow-up contact within 24 business hours post-discharge
  • Gather and summarize data for intake assessments
  • Maintain strict confidentiality of youth and family information
  • Support compliance with state, federal, and Joint Commission standards
  • Assist with admissions paperwork, audits, quality assurance reviews, and special initiatives
  • Attend staff conferences and required training
