The Case Manager is responsible for managing patient care within a multidisciplinary treatment team while coordinating closely with: Therapists and physicians External mental health liaisons Referral sources and care providers Insurance companies Parents and guardians This role ensures accuracy in: Utilization review Care management Discharge planning Contract compliance Electronic health record documentation You will be the central point of information flow for both internal teams and external partners. You will help ensure that no detail is missed, no communication breaks down, and no family is left uninformed.
