About The Position

Position Purpose: The primary purpose of this position is, under the direction of the Regional Director of Census, to represent The Alverno at hospitals and facilitate seamless transfers of patients from hospitals to The Alverno. This role is critical to census development and ensures effective coordination of admissions, discharge planning, and transitions of care while supporting Trinity Health’s mission, vision, and values.

Requirements

  • Bachelor’s Degree in Healthcare related field, or an equivalent combination of education and experience
  • Knowledge of medical terminology is preferred.
  • Prior sales experience is a plus.
  • Strong knowledge of skilled nursing admissions processes and regulations.
  • Understanding of Medicare, Medicaid, and managed care insurance methodologies.
  • Demonstrated ability to work independently with minimal supervision and exercise sound judgment.
  • Excellent written, verbal, and presentation communication skills.
  • Strong organizational, analytical, and problem-solving skills with the ability to manage multiple priorities.
  • Proficiency with Microsoft Word, Excel, PowerPoint, and electronic communication systems.
  • Ability to collaborate effectively in a shared leadership and virtual work environment.
  • Minimal travel (less than 10%) within assigned geographic region may be required.

Responsibilities

  • Represent THSC communities in hospitals and serve as the primary liaison for patient transitions to skilled nursing facilities.
  • Collaborate with acute-care hospital discharge planners, physicians, case managers, patients, and families regarding potential admissions.
  • Develop and maintain strong referral relationships and identify new referral sources to support census growth.
  • Review patient medical records and assess patients for appropriate level of care and financial viability prior to acceptance.
  • Conduct hospital rounds on potential admissions and on residents transferred to the hospital, reporting progress to facilities.
  • Participate in census calls and support community admissions teams within assigned geographic areas.
  • Analyze data, anticipate trends, and develop strategies to meet and exceed organizational census targets.
