About The Position

PCS is seeking a Hospital Liaison / Restoration Counselor to serve as the primary liaison between Piedmont Community Services, the geriatric state hospitals, and the children's state hospital (including but not limited to Catawba Hospital and Piedmont Geriatrics) and the Commonwealth Center for Children & Adolescents (CCCA). Responsibilities include providing discharge planning and coordination for patients from these facilities who reside in the Martinsville/Henry County, Franklin County, and/or Patrick County areas, ensuring they receive appropriate services and supports. The position will also assist in developing collaborative relationships with area nursing homes, assisted living facilities, and providers who can assist with guardianship appointments for patients needing this support. As a Restoration Counselor, the role includes delivering competency-restoration education, coordinating psychiatric and legal supports, and monitoring progress to ensure compliance with court-ordered restoration requirements in collaboration with mental health and legal partners. This position will serve out of the Franklin County office and report to the Hospital Liaison Lead.

Requirements

• One year experience of direct care of any equivalent combination of training and experience which provides required knowledge, skills, and abilities

Nice To Haves

• Bachelor's degree in human services or related field preferred

Responsibilities
