This is position is located at our West Valley Integrated Health Clinic! The Discharge Planning Specialist is a member of a collaborative person-centered care team that provides enhanced coordination with inpatient hospitals and emergency departments. This position is responsible for solidify services, supports and documentation needed to facilitate hospital discharge and rapid release from EDs and serves as the single point of contact for discharge coordination with the RBHA and involved stakeholders.
