About The Position

SuperCare Health (SCH) is a leading post-acute, in-home respiratory care provider in the Western U.S., serving patients for nearly 50 years. They specialize in respiratory and chronic disease management and have recently launched a Diabetes division. This position, the Hospital Liaison - Care Coordinator, is based in Carson, CA, and reports to the Patient Care Department Manager. The role involves building strong relationships within multiple medical facilities, providing prompt service, educating patients and families on equipment use, coordinating medical documentation, and actively promoting and marketing SuperCare Health’s full range of Durable Medical Equipment (DME), Respiratory therapies, and infusion services. In addition to patient care coordination, this role includes sales-related responsibilities such as identifying referral opportunities, strengthening relationships with healthcare providers, and increasing SuperCare Health's presence within assigned hospitals. SuperCare Health aims to be the most trusted and preferred resource to manage high-risk, post-acute respiratory patients through high-touch clinical teams and high-tech, innovative solutions, ultimately reducing hospital re-admissions, optimizing outcomes, and improving the lives of patients with chronic care diseases.

Requirements

  • Experience in medical supply, home medical equipment (HME), or related healthcare industries.
  • Strong knowledge of medical terminology and diagnosis codes.
  • Proven experience in customer service and relationship-building within a healthcare setting.
  • Ability to multitask, prioritize, and meet deadlines efficiently.
  • Strong organizational skills with attention to detail.

Responsibilities

  • Serve as the primary point of contact between SuperCare Health and hospital personnel, ensuring seamless coordination of DME and respiratory equipment deliveries to hospital rooms and patient homes.
  • Develop and maintain strong relationships with physicians, case managers, discharge planners, and other healthcare professionals to generate new business opportunities and drive referrals.
  • Promote and educate hospital staff on SuperCare Health's DME, respiratory therapies, and infusion services to increase awareness and referrals.
  • Conduct regular facility visits to ensure visibility and accessibility for healthcare professionals and patients.
  • Provide timely consultation to patients and their families regarding equipment delivery, potential co-pays, and usage instructions.
  • Request and ensure the accuracy of all necessary medical documentation for equipment orders.
  • Monitor and track referrals, ensuring SuperCare Health is the preferred vendor for partnered healthcare systems.
  • Identify opportunities to expand service utilization and sales within assigned hospital accounts.
  • Conduct or participate in presentations, in-services, and educational seminars for hospital staff and case managers to reinforce SuperCare Health’s value proposition.
  • Maintain accurate records of interactions with hospital personnel, patient referrals, and sales activities, submitting required reports in a timely manner.
  • Ensure compliance with insurance pre-authorization requirements and coordinate with Managed Care and Primary Medical groups as needed.
  • Maintain adequate inventory levels in assigned hospital storage areas to ensure prompt equipment availability as well as reconcile movement of equipment.
  • Collaborate with Customer Care Specialists and Dispatch teams to optimize equipment delivery and customer satisfaction.
  • Represent SuperCare Health professionally at all times and adhere to all HIPAA, company, and industry-related regulations.
  • Attend all department meetings and training sessions as scheduled.
  • Perform all other duties as assigned by department leadership.
