They specialize in respiratory and chronic disease management and have recently launched a Diabetes division. This position, the Hospital Liaison - Care Coordinator, is based in Carson, CA, and reports to the Patient Care Department Manager. The role involves building strong relationships within multiple medical facilities, providing prompt service, educating patients and families on equipment use, coordinating medical documentation, and actively promoting and marketing SuperCare Health’s full range of Durable Medical Equipment (DME), Respiratory therapies, and infusion services. In addition to patient care coordination, this role includes sales-related responsibilities such as identifying referral opportunities, strengthening relationships with healthcare providers, and increasing SuperCare Health's presence within assigned hospitals. SuperCare Health aims to be the most trusted and preferred resource to manage high-risk, post-acute respiratory patients through high-touch clinical teams and high-tech, innovative solutions, ultimately reducing hospital re-admissions, optimizing outcomes, and improving the lives of patients with chronic care diseases.
