Essential Duties: Under the general supervision of the Patient Care Department Manager, the Hospital Liaison - Care Coordinator plays a key role in cultivating strong relationships within multiple medical facilities. This individual is responsible for providing prompt service, educating patients and families on equipment use, coordinating medical documentation, and actively promoting and marketing SuperCare Health’s full range of Durable Medical Equipment (DME), Respiratory therapies, In addition to patient care coordination, this role includes sales-related responsibilities such as identifying referral opportunities, strengthening relationships with healthcare providers, and increasing SuperCare Health's presence within assigned hospitals.
