About The Position

Hospital-Based Hospice Liaison Named by The Boston Globe as one of the Top Places to Work in Massachusetts for the last four years, our community-based, mission-driven hospice and palliative care organization seeks a Hospital-Based Hospice Liaison to join its Community Development team. Our not-for-profit hospice and palliative care organization is seeking a self-starting, energetic, resourceful individual with excellent communication and interpersonal skills. This role is based at Newton-Wellesley Hospital. The Hospital-Based Hospice Liaison partners with hospital care teams to support timely access to hospice services, strengthen referral relationships, and facilitate smooth transitions from hospital to community-based care. The ideal candidate will be passionate about community outreach, education, and advocacy for hospice and palliative care, with a strong commitment to racial equity and bridging healthcare disparities. Schedule and Weekly Hours: Monday to Friday from 9 am to 5 pm. This is a 40-hour role.

Requirements

  • Minimum of 2 years of experience in healthcare, healthcare outreach, or a related field required.
  • Strong understanding of hospice care and end-of-life services.
  • Proven ability to build and maintain relationships with diverse stakeholders.
  • Excellent communication, presentation, and interpersonal skills.
  • Self-motivated, with strong organizational and time management skills.
  • Ability to work independently and as part of a team.
  • Knowledge of local community resources and healthcare providers.
  • Valid driver's license and reliable transportation.

Nice To Haves

  • Hospital-based experience strongly preferred.
  • Knowledge of or experience working in underserved communities and with marginalized populations.
  • Bilingual or multilingual abilities are a plus.
  • Proficiency in Google Suite applications (Docs, Sheets)
