About The Position

The Hospital at Home RN coordinates and evaluates complex care delivery projects, providing advanced clinical expertise, education, and contributing to research within their specialty. This role facilitates change and collaborates closely with Tele Hospitalists, Physicians, APPs, pharmacists, respiratory therapists, and patient placement teams to manage all aspects of hospital-level care at home admissions and transfers. The RN conducts virtual rounds to ensure adherence to evidence-based practices and proactively provides clinical leadership, mentorship, and support for resource pool and ancillary program nurses, driving high-quality outcomes in a dynamic care environment. Essential Functions Provides advanced clinical support and oversight for patients receiving hospital-level care in the home, including comprehensive virtual medication administration, patient education, and ongoing assessment of therapeutic effectiveness and safety. Serves as the central point of contact and clinical coordinator, triaging and resolving all incoming calls from patients, resource pool nurses, and ancillary departments, ensuring timely and effective communication and care escalation as needed. Develops, proposes, and refines individualized treatment plans, admission orders, and durable medical equipment (DME) needs, collaborating directly with providers to ensure optimal patient outcomes. Leads the coordination of all mobile diagnostic services (imaging, laboratory, and other ancillary orders), integrating results and clinical data into a unified patient care view for the interdisciplinary team. Manages the end-to-end logistics for provider and admission order sets, proactively identifying and resolving barriers to timely initiation of hospital-level care in the home setting. Serves as the clinical authority and definitive resource for complex problem-solving, offering expert guidance and support to ancillary services, providers, and patients to ensure high-quality, safe, and efficient care delivery. Functions as the definitive resource and consultant for all ancillary services, providers, and patients, offering expert guidance and support for clinical decision-making. Designs and delivers targeted education for patients and families and provides ongoing educational opportunities for bedside caregivers and resource pool nurses to advance clinical competencies in virtual and home-based acute care. Exercises independent clinical judgment and is empowered to make urgent care decisions within established protocols, including initiating rapid interventions, escalating care, and activating emergency response as clinically indicated.

Requirements

  • Current Registered Nurse (RN) license in state of practice.
  • Valid compact licenses must be transferred to the state of practice within 60 days of employment.
  • Bachelor of Science in Nursing (BSN) required or must be obtained within four (4) years of hire or promotion date. If there is an existing education agreement, that agreement will take precedence.
  • Basic Life Support (BLS) Certification for Healthcare Providers.
  • Demonstrated expert medical surgical nursing experience.
  • Proven clinical leadership experience, such as serving as a charge nurse, preceptor, or in a formal mentoring capacity.
  • Proven ability to coordinate complex care, facilitate interdisciplinary collaboration, and lead quality improvement initiatives.
  • Demonstrated ability to work independently and exercise a high degree of autonomy, including making complex clinical decisions and prioritizing care in a dynamic, remote environment.
  • Strong communication, critical thinking, and problem-solving skills, with a track record of effective decision-making.

Nice To Haves

  • Bachelor’s degree in nursing (BSN) from an accredited institution.
  • Experience with virtual care delivery, Hospital at Home programs, or advanced care coordination.
  • Certification in Case Management, Care Coordination, or a relevant specialty area.
