At Lifted Hospice, our Team Coordinators are the steady heart of our operations ensuring that nothing falls through the cracks as we care for patients and families in their most sacred season. As a Team Coordinator, you keep the flow of care organized, accurate, and timely. From scheduling and communication to document management and supply coordination, you play a vital behind-the-scenes role in helping the team deliver compassionate, efficient care. Your attention to detail and commitment to excellence helps every nurse, social worker, chaplain, and provider focus on what matters most: supporting patients with peace, dignity, and presence. As a Team Coordinator, you serve as the administrative backbone of the interdisciplinary team. You manage schedules, organize communication, and ensure clinical documentation is accurate, timely, and accessible. You help prepare for meetings, maintain patient records, coordinate supplies and equipment, and respond quickly to needs as they arise. You’re a calm, dependable presence who brings structure to fast-moving days keeping the team aligned and empowered to serve patients with excellence. Your work protects the continuity of care and ensures that every detail supports our mission of helping people live life to the fullest.
