Requirements

  • High school graduate
  • Proficiency in Microsoft Office.
  • Exceptional organizational and multitasking skills.
  • Excellent telephone and customer service skills.
  • Strong work ethic and time management skills.
  • Demonstrates patience, flexibility, and a cooperative attitude.
  • Comfortable participating in weekend and telephone on-call rotation.

Responsibilities

  • Coordinate the SOC visit by interacting with the patient or their representative.
  • Proactively plan and assign patients to clinical staff.
  • Perform visual checks of forms for discrepancies and process discharge documentation in a timely manner.
  • Maintain accurate logs and distribute reports to appropriate staff.
  • Update weekly schedules for interdisciplinary team staff and notify the Clinical Team Manager of discrepancies.
  • Verify visits provided against visits ordered and scheduled.
  • Receive and relay messages for assigned interdisciplinary team staff.
  • Prepare schedules for weekend and on-call coverage and order medical supplies as needed.
  • Participate in on-call rotation for weekend coverage and answer phones after hours.
  • Support the Agency's customer service program by maintaining a responsive and knowledgeable approach to customer contacts.
  • Perform other activities as assigned by the Clinical Team Manager.
