The Spiritual and Grief Counselor serves the interdisciplinary team by responding to referrals, developing and carrying out individualized care plans, and providing individual and group spiritual and grief support. Requirements

  • Degree in Social Work, Counseling (or other related counseling degree), Divinity, Theology, or Pastoral Care, or equivalent combination of education and work experience required
  • At least one unit of Clinical Pastoral Education (CPE); (required unless degree in Social Work obtained)
  • Valid driver’s license, auto insurance, and access to a reliable vehicle
  • Experience providing counseling and/or spiritual care in both a group and individual setting
  • Experience working with grief as related to death
  • Intermediate Microsoft Office Suite skills
  • Ability to work effectively and cooperatively with others
  • Experience using industry specific software
  • Ability to communicate effectively verbally and in writing
  • Ability to be flexible in time and task to meet organizational needs
  • Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
  • Ability to work at various Agrace locations
  • Ability to use proper body mechanics and standard precautions

Nice To Haves

  • Two or more years’ counseling experience preferred

Responsibilities

  • Respond to referrals from clinical teams by contacting the patient and/or family to establish counseling relationship and expectations.
  • Perform spiritual and grief assessment, intervention, and care planning to meet the individual needs of patients and/or families.
  • Engage in ongoing assessment of care plans through interdisciplinary review in accordance with Federal and State Guidelines
  • Work collaboratively with the patient’s designated faith and spiritual community, and refer to other community resources as needed
  • Develop an individualized bereavement plan of care to address the full scope of survivor needs.
  • Provide updates to the interdisciplinary team regarding bereaved
  • Connect bereaved with community resources to assist in the grieving process
  • Develop, implement, and facility support groups for Agrace families/community members
  • Provide supervision and support to counseling services volunteers
  • Participate in the planning and coordination of Agrace community bereavement events
  • Participate in internal work groups, meetings and committees relative to position responsibilities
  • Provide counseling support (through group sessions, memorial services, reflections, etc.) to the interdisciplinary teams and community partners
