Seasons Hospice is seeking a compassionate Hospice Social Worker to join our interdisciplinary care team. In this role, you will help patients and families navigate the emotional, social, and practical challenges that come with serious illness, ensuring they receive care with dignity, compassion, and support. Requirements
- Licensed Social Worker
- At least 1 year of healthcare experience (hospice preferred).
- Understanding of hospice philosophy and end-of-life care.
- Strong communication and counseling skills.
- Valid driver's license and reliable transportation
Responsibilities
- Assess psychosocial needs of patients and families.
- Provide counseling and emotional support during end-of-life care.
- Assist with advance directives and care planning.
- Connect families with community resources and services.
- Collaborate with nurses, physicians, and the interdisciplinary hospice team.
- Help families navigate difficult decisions with compassion and guidance.
