About The Position

Join our compassionate healthcare team as a Hospice Registered Nurse (RN), where your expertise will provide comfort, dignity, and personalized care to patients facing life-limiting illnesses. In this vital role, you will deliver comprehensive nursing services in patients’ homes and community settings, ensuring seamless coordination of care while supporting families through challenging times. Your dedication will help improve quality of life, manage symptoms effectively, and uphold the highest standards of hospice and palliative medicine. Duties Conduct thorough patient assessments including vital signs, symptom management, and physiological evaluations to develop individualized care plans aligned with hospice standards. Administer medications, including pain management therapies such as opioids and other symptom control medications, following strict protocols and safety procedures. Provide skilled interventions such as IV infusion therapy, wound care, suturing when necessary, ostomy care, and catheterization to maintain patient comfort and safety. Collaborate with interdisciplinary teams utilizing EMR (Electronic Medical Record) systems like Epic or Cerner to document patient progress accurately and ensure continuity of care. Educate patients and families on disease processes, medication administration, advanced directives, and end-of-life planning to foster understanding and emotional support. Coordinate with physicians for diagnostic evaluations including laboratory testing, imaging review (such as sonography or radiology reports), and medication adjustments based on clinical findings. Participate in triage activities to prioritize urgent needs, manage acute pain episodes, and respond promptly to changes in patient conditions across diverse settings such as assisted living facilities or nursing homes. On call rotation plus week visits as needed. PRN Visit Rates are Routine- Visit Rate ($65) plus Hourly Rate End of Life - Visit Rate ($75.00) Admit - Visit Rate ($125.00) On-call hourly rate ($3.75) hour weekly/ ($5.00) hour weekends Qualifications Valid RN license with current state registration; experience in hospice & palliative medicine preferred. Demonstrated critical care experience including ICU or emergency medicine backgrounds; familiarity with Level I & II trauma center protocols is advantageous. Proficiency in EMR/EHR systems like Epic or Cerner; experience with ICD-9/ICD-10 coding for accurate documentation. Strong knowledge of physiology, anatomy, pharmacology related to pain management, sedation techniques, and aseptic procedures. Skilled in medical documentation including CPT coding and medical records management; experience with medical imaging or laboratory processing is a plus. Ability to perform physical examinations, venipuncture (phlebotomy), IV insertion, sterile processing techniques, and specimen collection efficiently. Compassionate communicator with experience working with diverse populations such as geriatrics, memory care residents, individuals with developmental disabilities or dementia care needs. Certifications in Basic Life Support (BLS) required; additional certifications like ACLS or PALS are beneficial.
