About The Position
Puget Sound Home Health & Hospice has been providing life-changing services for over 25 years and now we are growing again! We are looking to add an experienced and caring Registered Nurse (RN) Hospice Clinical Manager for our Hospice division in Pierce County , Washington! Requirements
- A minimum of 1 year of health care management experience
- Must have at least 1 year of hospice agency experience, including admissions
- Must be experienced in HIS charting requirements for hospice
- Current BLS certification
- Current Washington state Registered Nurse (RN) license
- Knowledge of hospice regulations, including Medicare COPs, PDGM, VBP
- Licensed and insured driver with reliable transportation
Nice To Haves
- A BSN degree is preferred, but not required
- Experience with Home Care Home Base preferred, but not required
Responsibilities
- Ensures agency compliance with all state/federal hospice laws & regulations
- Reviews all incoming referrals for appropriateness & manages approvals & assignments
- Reviews patient charts for medical diagnoses, procedures, medications, & clinical course
- Assists the DCS with planning, implementing & evaluating in-services & CEU programs
- Assists with interviewing, hiring, training, & mentoring organization personnel
- Participates in QAPI, OASIS review, and coding workflow
- Provide symptom management and comfort care for hospice patients in their home
- Must be willing to take call when needed
