Hospice RN Case Manager – Defined Territory + Structured Caseload | Canonsburg Area If you've worked in hospice or home health, you know the difference between a role that's manageable—and one that constantly feels like you're playing catch-up. This opportunity is designed for an experienced RN who wants ownership of their patients, a structured caseload, and a team that actually supports the work behind the scenes . You'll be working within a defined territory around Canonsburg and surrounding communities , allowing you to focus on patient care without excessive travel.
