Concordia Hospice of Washington is a partnership between Concordia Community Support Services and Washington Health System that offers both quality home care and inpatient hospice end-of-life care. Concordia Hospice of Washington serves our patients in Washington and Greene counties with the focus to provide services that offer comfort, peace, and dignity for those with incurable illnesses and for their families. We strive to hire excellent people at Concordia - and keep them as well! Are you a Registered Nurse who believes in the power of comfort, dignity, and a personalized approach to end-of-life care? Concordia Hospice Of Washington is seeking a dedicated and personable PART-TIME Hospice RN to join our team and provide compassionate care to patients and families during this significant time in their lives. This role is part-time with future full-time potential. We are seeking two clinicians. One serving the Northern territory (Canonsburg and surrounding areas) One serving the Southern territory (Washington/Greene County).

Requirements

• Adaptability, compassion, and a commitment to patient-centered care.
• Ability to work independently and manage time effectively.
• A minimum of 2 years of nursing experience, preferably with experience in palliative care, hospice, or oncology.

Responsibilities

• Assessing and managing patient needs: Continuously assess the physical, emotional, and spiritual needs of patients, developing individualized care plans with the interdisciplinary team.
• Providing expert symptom management: Administer medications and treatments, focusing on pain management and symptom control to improve the patient's quality of life.
• Educating and supporting families: Be a trusted resource for families, providing education about the disease process, care options, and end-of-life decisions, while offering emotional support.
