About The Position

Are you a licensed RN looking for a rewarding opportunity in the Home Health Care industry? Cornerstone VNA is seeking a Full-Time Hospice RN to join our compassionate team. In this role, you'll provide comfort, dignity, and support to patients and their families during a very meaningful stage of life.

Requirements

• Current RN license in New Hampshire (or a compact state license)
• Valid driver's license and vehicle insurance
• Strong communication and interpersonal skills
• Compassion, empathy, and commitment to quality care
• Ability to adapt quickly to patient needs and medical changes
• Excellent organization and time management skills

Responsibilities

• Provide skilled nursing care to patients in the comfort of their own homes
• Coordinate and deliver care that meets each patient's unique needs and goals
• Adjust care plans in collaboration with physicians and the Hospice Care Team
• Support patients and families in fulfilling end-of-life wishes
• Ensure comfort, dignity, and emotional support during each visit
• Work independently while knowing support is just a phone call away
• Collaborate with our multi-disciplinary team to provide holistic care
