About The Position

As a Hospice RN Case Manager at Trinity Health At Home, you are fully responsible for the client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes, and are designated as the case manager for hospice patients. You will collaborate with physicians and other Hospice team members to anticipate and manage patient’s symptoms, assist in identifying patient/family/caregiver grief/loss issues, and assist in implementing the Hospice plan of care. Requirements

  • Active RN license in Michigan
  • 1+ years of clinical nursing experience
  • Strong communication, assessment, and organizational skills
  • Compassionate, dependable, and mission-minded
  • Valid driver’s license and reliable transportation required
