The Onboarding & Education Specialist (RN) at Transitions Care is a multistate clinical education professional responsible for designing, facilitating, and continuously improving corporate new hire orientation for all clinical and non-clinical hospice staff across the organization. This role serves as the primary point of accountability for ensuring consistent onboarding experiences that align with Medicare Conditions of Participation, state-specific regulatory requirements, and organizational standards of care. In addition to orientation facilitation, this position monitors and reports on employee education completion rates across the Learning Management System (LMS), identifies compliance gaps, and partners with branch leadership to drive timely course completions.

Requirements

  • Current, unencumbered Registered Nurse (RN) licensure required; multistate Nurse Licensure Compact (NLC) licensure strongly preferred
  • Associate’s Degree in Nursing required; Bachelor of Science in Nursing (BSN) preferred
  • Minimum 3 years of clinical experience in hospice or home health required
  • Minimum 2 years of experience in staff education, training, or organizational development preferred
  • Experience facilitating adult learning programs in a healthcare setting required
  • Thorough knowledge of Medicare Hospice Conditions of Participation (42 CFR Part 418), HQRP quality measures, and hospice billing and compliance requirements
  • Familiarity with adult learning principles (Knowles’ andragogy), instructional design methodologies, and competency-based education
  • Strong facilitation and public speaking skills with demonstrated ability to engage diverse audiences in both in-person and virtual settings
  • Proficiency with Microsoft Office Suite (Word, Excel, PowerPoint, Teams); experience with LMS platforms (e.g., Relias) required
  • Highly organized with strong project management skills and ability to manage multiple competing priorities across geographic locations
  • Strong written and verbal communication skills; ability to translate complex regulatory requirements into accessible, actionable training content
  • Ability to work independently with minimal supervision in a remote/hybrid environment while maintaining strong cross-functional collaboration
  • Commitment to equity, inclusion, and culturally responsive education practices

Nice To Haves

  • Certification in Hospice & Palliative care (CHPN) preferred but not required

Responsibilities

  • Design, coordinate, and facilitate multistate corporate new hire orientation for all hospice disciplines including RNs, LPNs, social workers, chaplains, hospice aides, and administrative staff
  • Deliver live (in-person and virtual) orientation sessions that integrate Medicare Conditions of Participation (42 CFR Part 418), HQRP quality measures, organizational policies, and state-specific regulatory requirements across all operating states
  • Develop and maintain orientation curriculum, facilitator guides, participant workbooks, and competency validation tools, updating content in response to regulatory changes, survey findings, or organizational initiatives
  • Collaborate with the Compliance, Clinical, and HR departments to ensure orientation content reflects current standards, including OSHA, HIPAA, infection control, emergency preparedness, and hospice-specific clinical competencies
  • Coordinate scheduling logistics for orientation cohorts across multiple states, working with branch administrators and hiring managers to ensure timely onboarding of new employees
  • Administer post-orientation competency assessments and skills validations; document results and escalate concerns to appropriate supervisors
  • Serve as a resource and subject matter expert for preceptors and field supervisors supporting new employees during their orientation period
  • Monitor and track employee completion rates for all mandatory and elective education courses within the organization's Learning Management System (LMS) on an ongoing basis
  • Generate and distribute weekly, monthly, and quarterly education compliance reports to branch administrators, regional directors, and senior leadership, highlighting completion rates, delinquencies, and trends by location, discipline, and course
  • Identify employees and departments with incomplete or overdue required training and implement targeted follow-up strategies in collaboration with branch leadership to achieve timely resolution
  • Establish and maintain dashboards or tracking tools that provide real-time visibility into organization-wide education compliance status across all operating states
  • Assign, update, and troubleshoot course assignments, curricula, and learner records; ensure new hires are enrolled in required tracks upon hire
  • Conduct root cause analysis on systemic completion gaps and develop corrective action plans in collaboration with branch and regional leadership
  • Support survey preparedness by ensuring education records and completion documentation meet regulatory requirements and are audit-ready at all times
  • Assess organizational learning needs through surveys, quality data, QAPI findings, survey deficiencies, and consultation with clinical and compliance leadership
  • Develop new education programs and update existing content in response to identified gaps, regulatory updates, or emerging best practices in hospice and palliative care
  • Evaluate the effectiveness of education programs through post-training assessments, competency outcomes, quality metrics, and learner feedback; use data to drive program improvements
  • Maintain current knowledge of hospice regulatory requirements across all states in which the organization operates, including Medicare CoPs and applicable state licensure regulations
  • Represent the Education department in interdisciplinary meetings, QAPI committees, and corporate initiatives as requested
  • Field Support in time of staffing crisis as determined by the Chief Nursing Officer
  • Adherence to a code of conduct conducive with Transitions Hospice policy is expected.
