The Hospice Nurse provides skilled nursing care to patients at the end of life, conducting visits in facilities and/or patient homes. This role includes providing on-call services after hours and completing scheduled skilled visits. Responsibilities involve responding to phone calls from patients and families regarding symptoms, condition changes, or emergencies, and conducting home/facility visits to assess critical symptoms like pain management, respiratory distress, or emotional crisis. The nurse assists the Registered Nurse Case Manager (RNCM) in carrying out the plan of care and helps the nursing team perform specialized procedures as directed by the RNCM under physician's orders. Active participation in the Interdisciplinary Group Huddle (IDGH) is required to ensure the patient's plan of care is current and issues are addressed. The position also involves reinforcing patient and family education on end-of-life care, assuming responsibility for personal and vocational growth through professional participation and learning, keeping the RNCM informed of patient changes, and assisting with emergent visits including continuous care and companion care as needed.
