As a Hospice Nurse (LVN) with Touchstone Health in San Antonio, you will provide deeply human work, offering presence, steadiness, and support to patients and families during end-of-life transitions. Requirements

  • Current, unrestricted LVN license in Texas
  • At least 1 year of nursing experience
  • CPR (BLS) certification
  • Ability to remain calm under pressure and provide steady, reassuring care
  • Strong reliability and follow-through — patients and families depend on you
  • Comfort working independently in the field while staying connected to a team
  • Valid driver’s license and reliable transportation

Responsibilities

  • Provide skilled nursing care to hospice patients under the direction of an RN, following individualized care plans
  • Administer medications and treatments while ensuring patient comfort and symptom management
  • Monitor and document physical, emotional, and psychosocial changes, communicating updates to the care team
  • Educate patients and families on care, expectations, and comfort-focused approaches
  • Collaborate with interdisciplinary team members to provide coordinated, compassionate care
  • Maintain accurate, timely documentation and uphold all compliance and safety standards
