About The Position

Medical Social Worker (Master's Level MSW) Emotional support. Resource guidance. Advocacy. The Care Team Hospice is seeking a part-time or PRN Medical Social Worker (Master's Degree & Master's License) to help support patients and families through compassionate guidance and meaningful connection during the hospice journey. The Medical Social Worker provides psychosocial support to hospice patients and families receiving care in home or facility settings, helping address emotional, social, and practical needs throughout the course of care. Schedule, Hours & Territory • Hours/Schedule: Standard operating hours are Monday through Friday, 8:30 AM to 4:30 PM; there may be flexibility to work evenings/weekends occasionally • Territory: Covers a large multi-county service area across central Indiana. This PRN role offers flexibility, and patient visits are assigned based on location to create efficient, localized routes rather than requiring daily travel across the full territory. Travel expectations vary based on census and location, but efforts are made to keep visits within a reasonable geographic area per shift. Requirements

  • Master’s degree in Social Work and an active Master's Level Social Work License required
  • Active driver’s license
  • Ability to pass a criminal background check
  • Ability to pass a Motor Vehicle Record (MVR) check
  • Current TB testing in accordance with agency policy
  • Current CPR certification as required for the role

Nice To Haves

  • Hospice experience preferred
  • Experience using HCHB (Homecare Homebase) preferred

Responsibilities

  • Conduct psychosocial assessments
  • Provide counseling and emotional support to patients and families
  • Assist with advanced care planning and coordination of community resources
  • Participate in interdisciplinary team meetings
  • Document services accurately and timely in the EMR HCHB (Homecare Homebase)
