About The Position

Hospice Medical Social Worker Emotional support. Resource guidance. Advocacy. (Master's Level License Required) Looking for an MSW role where your impact actually matters? We’re hiring a Medical Social Worker to support hospice patients and families with compassionate, real-world guidance through every stage of care. Schedule, Hours & Territory • Standard operating hours are Monday through Friday, 8:30 AM to 4:30 PM • Reading, PA and surrounding area • Participation in an on-call rotation may be required, with additional compensation provided What We Offer • Meaningful role supporting patients and families emotionally and socially • Strong interdisciplinary team collaboration • Additional compensation for on-call participation • Mission-driven hospice environment

Requirements

• Master’s degree in Social Work required
• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role

Nice To Haves

• Hospice experience preferred
• Experience using HCHB (Homecare Homebase) preferred

Responsibilities

• Conduct psychosocial assessments
• Provide counseling and emotional support to patients and families
• Assist with advanced care planning and coordination of community resources
• Participate in interdisciplinary team meetings
• Document services accurately and timely in the EMR HCHB (Homecare Homebase)
