As an Evening Hospice Licensed Practical Nurse (LPN) at Elara Caring, you’ll serve as a trusted clinical presence for patients receiving hospice services at home. This role is designed for nurses who value connection, consistency, and purpose-driven care—without overnight shifts or weekend rotations. --------------------------------------------------------------------------- The Role at a Glance In this field-based position, you’ll partner closely with Registered Nurses and the interdisciplinary hospice team to deliver hands-on nursing care that supports comfort, safety, and dignity. Requirements

  • Graduation from an accredited practical nursing program
  • Active LPN license in the state of practice
  • At least one year of hands-on clinical nursing experience (Hospice, palliative, or home-based care experience is a plus)
  • Strong clinical judgment and comfort working independently in the field
  • Clear, empathetic communication skills
  • Organized, dependable, and able to manage time efficiently
  • Willingness to travel within the service area (up to 50%)
  • Reliable transportation and ability to meet agency health and screening standards

Nice To Haves

  • Hospice, palliative, or home-based care experience is a plus
  • Highly preferred to live in the service area of Lenawee, Hillsdale, and or Monroe Counties

Responsibilities
