Responsibilities

  • Demonstrates knowledge of applicable laws and regulations, developmental stages, pharmacology, health promotion and disease prevention and treatment, best practice interventions, Baystate Home Health and Hospice policies and procedures through daily interactions with staff, physicians, and patient/families
  • Receives phone calls and directs triage to ensure that patient's needs are prioritized and met, which may include an unscheduled visit or wellness calls
  • Receives and processes patient referrals, develops electronic medical records and collects necessary documentation to maintain regulatory compliance for a hospice admission; facilitates and complete communication as needed between patients/families, providers, and hospice team members; and provides educational and supportive information to patients and families related to hospice services and admission processes
  • Review reports and patient activity to prioritize admissions and routine visits. Work with management and team assistants to ensure efficient use of all staff with manager/clinical support and oversight.
  • Collaborates with finance and billing to ensure accurate payor source information is in electronic medical record
  • Assists in daily reports and communication of patients' needs to after-hours shifts.
  • Enter/edit/and/or process physician orders as appropriate in electronic medical records for updates in medication, equipment, scheduling, and plan of care needs.
  • Completes weekend scheduling in collaboration with clinical manager
  • Completes urgent visits as needed
  • Serves as a role model for effective communication across the organization. Supports clinical managers in visiting staff accountability
  • Coordinates smooth transitions of care for hospice patients between hospitals, homes, facilities, and other hospices.
  • Supports clinical staff with coordination of patient care needs including and not limited to DME, medications, supplies
  • Monitors utilization of resources and reports to the clinical manager
  • Assures accuracy of documentation in EMR including but not limited to visit times, orders, physicians, appropriate attachments, CIS accounts for the General Inpatient program
  • Timely communication with contracted vendors for patient services as needed, including enteral feeding, infusion services, etc.
  • Assumes a role in assisting with the development and implementation of policies, procedures, and standards of care for hospice patients.
  • Assumes accountability for own professional practice within ethical and legal parameters of nursing.
  • Works collaboratively with clinical staff and clinical manager to ensure timely communication within the team, referral sources, and throughout transitions of care
  • All other duties as assigned
