Hospice On-Call Licensed Practical Nurse: Hospice On-Call Licensed Practical Nurse Concordia Hospice of Washington is seeking a compassionate and dependable On-Call Licensed Practical Nurse (LPN) to provide comfort-focused care and support to patients and families outside of regular business hours. This role is ideal for a nurse who is confident working independently and committed to delivering meaningful, patient-centered care. This position requires travel throughout the South Hills region and Washington County. Requirements

• A Licensed Practical Nurse with a current, active license
• Calm, confident, and comfortable working autonomously

• Comfortable traveling within assigned territories

Responsibilities

• Respond to patient needs during evenings, nights, and weekends
• Provide skilled nursing care focused on comfort and symptom management
• Assess patient conditions and communicate updates to the care team
• Offer education, reassurance, and guidance to patients and families
• Deliver care with compassion, professionalism, and sensitivity
• Represent Concordia Hospice of Washington with empathy and respect
