Licensed Practical Nurse - Hospice On-Call
About The Position
First Choice Hospice is looking for a full time Hospice On-Call Licensed Practical Nurses (LPN / LVN) to join our dedicated team to provide community-based care for our patients currently receiving home health services in the Boise, ID metro area. Choose the schedule that fits your life! Full time - 7 on / 7 off nights orMonday - Thursday 4:30 PM to 8:30 AM The primary function of the LPN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.
Requirements
- Education: Graduate of an accredited School of Vocational/Practical Nursing.
- Licensure: Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license.
- Experience: Two years’ experience as a Licensed Vocational/Practical Nurse in a clinical care setting.
- Skills: Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills.
- Transportation: Reliable transportation and valid, current auto liability insurance.
Nice To Haves
- Home health experience highly preferred.
- EMR system: HCHB experience highly preferred
Responsibilities
- Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.
- Coordinate care with the interdisciplinary group, patient/family, and referring agency.
- Assist in the development and implementation of patient care plans.
- Monitor patient health and report any changes to the supervising RN.
- Administer medications and treatments as prescribed.
- Educate patients and their families on health care needs, conditions, and options.
- Maintain accurate and timely documentation of patient care and progress.
