Hospice Liaison Day (United States of America) Hospice Liaison Promotes Halifax Health, Health services (hospice, palliative and supportive care) in the community through educational in-services. Serves as a liaison with physicians’ offices, health care facilities, hospitals and community groups. Provides education regarding continuum of care services (i.e. hospice, palliative and supportive care). JOB QUALIFICATIONS: Knowledge of Hospice philosophy of care. Medicare conditions of participation; State of Florida regulations governing hospice; hospice operations and sales and marketing. Healthcare experience, medical sales and marketing experience preferred. Good written, verbal and public speaking skills required. Possession of valid Florida driver license and clear driving record. Bachelor’s degree in related field or higher or equivalent years of experience. DUTIES AND RESPONSIBILTIES : Establish and maintain good working relationships with referral sources to include, but not be limited to: physician, hospital case manager, nursing home staff, assisted living facility staff and other allied healthcare professionals. Educate and be an ongoing resource of skilled care and end-of-life care and services offered by Halifax Health-Hospice and to the medical and non-medical community. Educate and be an ongoing resource of Halifax Health system services. Develop plans of care for referral sources based upon referral practices and identify barriers to early referral. Develop and maintain good customer knowledge base. Act as liaison between referral sources and clinical operations of health system. Provide feedback to ensure optimal customer satisfaction and patient outcomes. Ability to document customer compliments and complaints into the Halifax EMS system.
