About The Position

At Riverside Home Health & Hospice, we have been serving patients and families in Southern Oregon for over 20 years. In recent years, we have expanded our Hospice services — providing comfort, dignity, and compassionate support during life’s most sacred moments. Every hospice journey begins with one call. We are seeking a Hospice Intake Coordinator who understands that how we respond in that first moment truly matters. This role plays a critical part in ensuring patients and families feel supported, informed, and cared for from the very beginning.

Requirements

• Strong communication and organizational skills
• Ability to manage multiple priorities with urgency and accuracy
• Healthcare, hospice, admissions, or intake experience preferred
• Attention to detail and strong documentation skills
• Self-directed and comfortable working independently
• Ability to collaborate effectively across teams

Responsibilities

• Receiving and reviewing hospice referrals
• Speaking with patients and families to gather information and explain services
• Coordinating with physicians and referral partners
• Confirming eligibility documentation and required orders
• Scheduling admissions and coordinating clinician assignments
• Ordering durable medical equipment (DME), medications, and supplies
• Supporting smooth transitions from hospital or facility to home
• Identifying and resolving barriers prior to admission
• Maintaining accurate and timely documentation
