We are seeking a compassionate and experienced Hospice Licensed Practical Nurse (LPN) to serve as our Intake Coordinator to manage the admissions process, working closely with clinical staff, patients, referral sources, and the business development team. This role ensures timely communication and coordination of patient care, following agency standards. You’ll serve as a patient care advisor and liaison between Hospice Intake Coordinators, Business Development Team liaisons clinical staff, physicians, and patients.
