Schedule: Monday through Friday 8:00 am to 4:30 pm Company: Redcrest Hospice HOW YOU'LL MAKE A DIFFERENCE: Care and compassion are at the heart of what you'll do as a Hospice, General In-Patient Nurse-Case Manager. You'll help deliver care in a holistic, family-centered manner anticipating, preventing and treating undesirable symptoms or secondary symptoms. HOW YOU'LL WORK : You'll be responsible for providing end-of-life care and Hospice services to patients in the Health System Inpatient Hospice setting in a manner consistent with standards of practice. You will assist in determining the need to change the level of care based upon assessment and collaboration with the Attending Physician or Medical Director.

Requirements

  • Registered Nurse within the state of employment.
  • Minimum 3 years inpatient clinical experience, preferably hospice.
  • Valid driver's license and auto insurance in your name as a driver .
  • Capable of all physical demands.

Nice To Haves

  • Bachelor's Degree in Nursing preferred.

Responsibilities

  • Plan of Care: Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure and effective grieving for patient and family.
  • Family Educator/Advocate: Maintain the dignity of the dying patient while supporting their unique spiritual and cultural beliefs, provide appropriate support at the time of death, including disposition of medications, notification of funeral home, etc. according to agency policy.
  • Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Communication: Provide referral sources with information on hospice programs and the coordination of patient care for optimal transition from the hospital to discharge destination in a hospice care setting wherever the patient calls home.
  • Operations: Attend interdisciplinary team meetings for full team discussions, consultation and evaluation of patient/family needs.
