Registered Nurse (RN) Hospice Case Manager
About The Position
The Registered Nurse (RN) Hospice Case Manager works in collaboration with the Interdisciplinary Discipline Team (IDT) to foster a quality experience according to the patient and family’s care plan. The RN Case Manager plans and manages the nursing component of the hospice caseload. The RN Case Manager makes initial and ongoing nursing assessments, judgments and treatment decisions based on patient and family/caregiver needs. He/she will manage symptoms of the patient's disease and re-evaluate effectiveness of interventions and adjust the care plan as needed. Location: Washington, D.C. - Home Care Hours: Full-time: Monday - Friday 8:30am-5:00pm COVID-19 vaccine required to be completed upon start.
Requirements
- Must have one year of staff nurse experience in a comparable setting/unit.
- Must have a degree in Nursing from an accredited nursing program.
- Must have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from compact state is acceptable.
- Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia.
- Must have a driver's license in good standing and access to a vehicle during working hours.
Nice To Haves
- Hospice and/or palliative care experience is strongly desired.
- A BSN degree is preferred.
- A CHPN certification is a plus.
Responsibilities
- Provides, coordinates, teaches and supervises the nursing care of Hospice patients and families/caregivers.
- Responsible for supervising Licensed Practical Nurses, Certified Nursing Assistants, and Patient Care Volunteers.
- May be asked to rotate to other regions/departments.
