About The Position

The Hospice Clinical Liaison serves as a key connector between Hospice of the North Coast and the community. This position blends clinical knowledge with marketing, outreach, and relationship development. The Liaison is responsible for driving admission growth, increasing HNC's visibility, and strengthening referral partnerships across hospitals, skilled nursing facilities, assisted living communities, physician groups, and community organizations. Ideal candidates are licensed clinicians such as RN, LVN, LCSW, or MSW who bring strong communication skills, clinical understanding, and a natural ability to build trust. This role requires initiative, professionalism, and a passion for ensuring that patients and families receive timely access to quality hospice care.

Requirements

  • Licensed RN, LVN, LCSW, or MSW (required)
  • Two or more years of healthcare sales, outreach, clinical education, or community liaison experience
  • Excellent communication, public speaking, and relationship-building skills
  • Self-motivated with strong organizational and time management skills
  • Ability to work independently while supporting team objectives
  • Valid driver's license, reliable transportation, and current auto insurance
  • Strategic thinking and sound judgment
  • Passion for mission centered work
  • Strong work ethic with reliability and initiative
  • Skilled in marketing, networking, and cold calling
  • Effective and compassionate communication
  • Confident public speaking and presentation abilities
  • Highly organized with attention to detail

Nice To Haves

  • Strong understanding of hospice, palliative care, or acute care preferred
  • Bilingual English/Spanish a plus

Responsibilities

  • Grows market mix and obtains new referral sources for Hospice of the North Coast
  • Drives admission growth by meeting quarterly targets after the initial ramp up period
  • Conducts outside marketing calls to potential and existing referral sources
  • Generates and implements quarterly strategic plans to support consistent referral activity
  • Completes and documents call activity, CRM entries, and administrative assignments on time
  • Promotes and positions HNC's programs and services to assisted living communities, skilled nursing facilities, medical groups, civic organizations, and the public
  • Delivers educational in services with accuracy, professionalism, and confidence
  • Represents the mission, quality, and philosophy of HNC in all external engagements
  • Serves as a clinical resource to community partners, patients, and families
  • Builds and nurtures long term referral relationships through consistent, meaningful engagement
  • Collaborates with HNC's interdisciplinary team to ensure excellent care coordination
  • Provides ongoing customer service and support to assisted living and skilled nursing facilities
  • Assists with coordinating resident transitions and addressing partner needs in a timely manner
  • Introduces and educates industry professionals, families, and individuals about hospice and palliative care
  • Clearly communicates hospice eligibility, philosophy, and HNC's differentiators
  • Ensures that every interaction reflects the compassion and quality that define HNC
