About The Position

Astrana is seeking a CHHA (Certified Home Health Aide) to provide in-home personal care, assistance with daily living (bathing, dressing, mobility), light housekeeping, meal prep, medication reminders, and emotional support for patients, working under a nurse's supervision to help clients maintain independence and a safe environment. We are ramping up our hospice agency and we cannot do so until we have the right CHHA in place! We are looking for candidates who are very flexible with hours, as we anticipate things being slower for the first 6 months (average of 4 hours a week, up to 20 hours a week once we are at a full roster). We are looking for candidates who are comfortable driving within a 50 mile radius of Alhambra.

Requirements

Completion of a CHHA training program and certification.
Strong communication, patience, empathy, and reliability.
Ability to follow instructions and work with minimal supervision.
Basic knowledge of health care practices and safety.

Nice To Haves

Mandarin or Cantonese language skills is a plus!

Responsibilities

Personal Care: Assisting with bathing, grooming, dressing, toileting, and skin care.
Mobility Assistance: Helping patients move in and out of bed, walk, and use assistive devices.
Health Monitoring: Checking and recording vital signs (pulse, temperature) and reporting changes to supervisors.
Medication Support: Providing reminders and assisting with self-administered medications as directed.
Household Tasks: Light housekeeping, laundry, meal planning, grocery shopping, and preparing meals.
Companionship & Support: Offering emotional support, engaging clients in social activities, and fostering independence.
Documentation: Completing records of services, patient progress, and observation
Other duties as assigned.
