Per Diem Certified Home Health Aide (CHHA)

As a Certified Home Health Aide (CHHA), you’ll play a vital role in delivering one-on-one care that makes a real impact—every visit, every time. We’re looking for caring, dedicated professionals who want to make a meaningful difference by providing exceptional home care services across Los Angeles and surrounding areas. As a CHHA, you'll be providing essential personal care and support services in the comfort of your patients’ homes. You’ll follow an individualized care plan under the direction of a registered nurse or therapist, helping patients with daily tasks while preserving their dignity and independence.
Requirements
- Current CHHA Certification through the California Department of Public Health
- Reliable Transportation – You’ll be traveling to patients' homes from Calabasas, Studio City, Sunland, Chatsworth, San Fernando & Surrounding Areas
- CPR Certification – Current and valid
- Strong Communication Skills – Fluent in spoken and written English
- Empathy & Professionalism – Compassionate, respectful, and dedicated to patient-centered care
Nice To Haves
- Home Health Experience – Preferred, but not required
Responsibilities
- Personal Care Assistance: Help patients with bathing, grooming, dressing, and toileting
- Mobility Support: Assist with transfers, walking, exercises, and fall prevention
- Monitor Vital Signs: Check and document temperature, pulse, and respiration during each visit
- Meal Preparation: Assist with meals and feeding based on the patient’s dietary plan
- Medication Reminders: Prompt patients to take prescribed medications (no administration required)
- Light Housekeeping: Change bed linens, keep the environment tidy and safe
- Observe & Report: Monitor patient condition and communicate changes to clinical staff
- Follow Care Plan: Deliver care based on each patient’s personalized home health care plan
