Position Overview: We are currently seeking a skilled and empathetic Registered Nurse (RN) to join our team as a Hospice Case Manager. As a Hospice Case Manager RN, you will have the opportunity to provide holistic, patient-centered care, ensuring comfort and dignity for our patients during their end-of-life journey. Collaborating closely with our interdisciplinary group, you will play a vital role in coordinating and delivering comprehensive hospice services in alignment with the highest professional and ethical standards.

Requirements

  • Valid and active California Registered Nurse (RN) license.
  • Minimum of 1 year of experience in or with hospice, palliative care, or a related healthcare setting.
  • Comprehensive knowledge of end-of-life care, pain management, symptom control, and emotional support.
  • Strong assessment, critical thinking, and problem-solving skills.
  • Excellent communication and interpersonal skills, with the ability to empathize and provide compassionate care.
  • Proficiency in using electronic medical records (EMR) systems.
  • Reliable transportation and a valid driver's license.

Responsibilities

  • Conduct thorough assessments of patients' physical, emotional, and psychosocial needs to develop personalized care plans.
  • Coordinate and oversee the implementation of the care plan, ensuring the provision of quality, compassionate, and interdisciplinary care.
  • Monitor patients' conditions, identify any changes or concerns, and provide timely interventions and support.
  • Administer medications, provide symptom management, and perform skilled nursing procedures as needed.
  • Offer guidance and emotional support to patients and their families, addressing their concerns and promoting open communication.
  • Collaborate with the interdisciplinary team, including physicians, social workers, spiritual counselors, and therapists, to ensure holistic care.
  • Maintain accurate and up-to-date documentation using electronic medical records (EMR) systems.
  • Educate patients, families, and caregivers on the hospice philosophy, treatment options, and available resources.
  • Participate in regular team meetings, case conferences, and ongoing education to enhance clinical knowledge and skills.
