St. Mary's Home care & Hospice is seeking a full time Hospice RN Case Manager ​St. Mary’s Home Care and Hospice located in Bucks County, PA. provides compassionate, exceptional care where people are most comfortable: at home. Hospice RN Case Manager position summary Responsible for case managing a team of hospice patients. RN case manager will collaborate with interdisciplinary team and attend weekly Interdisciplinary meetings. RN Case manager will interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms. St. Mary’s Home Care and Hospice is in Bucks County, PA. and is a member of Trinity Health at Home, a national home care, palliative care and hospice organization serving communities in twelve states. Requirements

  • Graduate of an approved nursing education program
  • Licensure as a Registered Nurse in the State of Pennsylvania
  • 2 years experience as a professional nurse in acute care, home care or ER experience required

Nice To Haves

  • Home care or Hospice experience preferred but not required
  • EPIC experience preferred
