As a Hospice Care Transitions Nurse , you will serve as a trusted clinical guide—helping patients, families, and healthcare partners understand hospice services and ensuring a seamless, supportive transition into care. This role blends clinical expertise, relationship-building, and education , making a meaningful difference at a pivotal time in people’s lives. How You’ll Contribute Reporting to the Director of Admissions, the Hospice Care Transitions Nurse plays a critical role in welcoming patients into hospice care and ensuring continuity as care transitions to the interdisciplinary team (IDT). This role is focused on our relationships and patients at Mass General Brigham’s Newton Wellesley Hospital and Faulkner Hospital.

Requirements

  • Registered Nurse (RN), currently licensed in Massachusetts and in good standing
  • Minimum of three years of professional nursing experience , with at least one year in an acute care setting preferred
  • Strong assessment skills and the ability to respond compassionately to the needs of patients and families in diverse settings
  • Emotional resilience and comfort supporting individuals and families during times of stress and uncertainty
  • A solid understanding of hospice philosophy and end-of-life care principles
  • Ability to work independently while thriving as part of an interdisciplinary team
  • Valid driver’s license and reliable, insured vehicle in accordance with organizational requirements

Responsibilities

  • Conduct comprehensive patient assessments to determine eligibility and appropriateness for hospice care
  • Coordinate and complete hospice admissions, ensuring a smooth transition to ongoing care by the IDT
  • Educate patients, families, and healthcare professionals across care settings about hospice philosophy, services, and benefits
  • Serve as a clinical liaison to hospitals, skilled nursing facilities, physician practices, and community partners
  • Proactively identify new referral opportunities and strengthen relationships with existing referral sources
  • Provide educational programs and in-services to healthcare facilities
  • Collaborate closely with Care Dimensions clinical teams to enhance communication, continuity, and quality of care
