About The Position

Under the direction and supervision of the In-Home Hospice Manager (a Registered Nurse), the Hospice and Palliative Care Intake Nurse Lead is a key member of the Tri-Cities Chaplaincy team. This position is responsible for managing and coordinating the intake process for patients entering hospice and palliative care services. Primary responsibilities include assessing patients' needs, obtaining and reviewing relevant medical information, and ensuring a seamless transition from initial contact to assignment with the appropriate provider. The Nurse Lead also provides leadership, supervision, and guidance to the intake and triage nursing team, ensuring compliance with protocols, regulatory requirements, and the highest standard of patient-centered care.

Requirements

  • Valid and unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Washington.
  • Certification in Cardio-Pulmonary Resuscitation (CPR)/Basic Life Safety (BLS)
  • Must possess and maintain a valid driver’s license, as driving is an essential function of this role. Must meet insurance eligibility requirements.
  • At least two years of nursing experience, preferably with one year of acute care, home health, or hospice nursing.
  • Availability to work evenings, weekends, nights, and holidays.
  • This position is designated as safety-sensitive and will require pre-employment drug testing, which includes cannabis testing in WA State.
  • Ability to report to the assigned work location(s) as scheduled and maintain consistent, reliable attendance.
  • Participate in the administrator on-call nursing supervisor rotation; respond promptly and effectively to patient and employee needs, including providing direct patient care when necessary

Nice To Haves

  • Bachelor’s degree in nursing.
  • Two years of management/leadership experience.
  • Bilingual in Spanish and English, with the ability to speak, read, write, and communicate effectively in both languages.
