About The Position

The Hospice and Palliative Care Clinical Liaison Registered Nurse (RN) serves as a vital connection between patients, families, healthcare providers, facilities, and Tri-Cities Chaplaincy. This role is responsible for promoting access to hospice and palliative care services, ensuring timely and compassionate care coordination, and building strong relationships with referral sources and community partners. The Clinical Liaison RN provides clinical expertise, patient advocacy, and education while supporting the agency’s outreach and growth.

Requirements

Current, unrestricted, Washington State license as a professional registered nurse.
Expertise in written and oral communication and Electronic Health Record (EHR) documentation.
Ability to quickly adapt to changing situations and respond accordingly.
Ability to provide hospice education to facility staff members, patients, and families.
Willingness to travel within the community.
Availability to work flexible hours, including evenings, weekends, and holidays as needed.
Must have reliable transportation and the ability to independently drive within Tri-Cities Chaplaincy’s service areas to provide in-home patient and family services and support in clients' homes, facilities, and community locations.
Must possess and maintain a valid driver’s license, as driving is an essential function of this role. Must meet insurance eligibility requirements.
This position is designated as Safety Sensitive. Must be able to pass a pre-employment drug screen which includes cannabis testing in Washington State.
Certification in Cardio-Pulmonary Resuscitation (CPR)/Basic Life Safety (BLS)
Demonstrate support and adherence to the agency’s Mission, Vision, and Values.
Participate in education and outreach activities to promote agency programs, including hospice care, advanced planning, and bereavement services.
Maintain knowledge of and comply with agency policies, procedures, and practices.
Protect patient and agency information per HIPAA guidelines. Only access information on a need-to-know basis.
Maintain reliable personal transportation. Maintain a valid driver’s license and vehicle insurance per state requirements.
Satisfy state and agency health requirements by providing proof of immunization, declinations, or approved accommodations as applicable.
Pass criminal background and drug screenings.
Meet the physical and environmental requirements of the position, with or without reasonable accommodation.(see attached physical requirements)
Complete agency and departmental orientation and required training programs.
Complete ongoing education, policy reviews, and training be designated deadlines.
Maintain current licensure(s) and certifications(s) required for the role.
Read, write, speak, and understand English fluently.
Be available to work as assigned or scheduled.
