Nurse Practitioner - Hospice & Palliative Care

You will go home knowing you made a positive impact in a patient’s life. The Nurse Practitioner has a 20hr/week (2-10hrs shifts) schedule and participates in the on-call weekend rotation. HopeWest has a unique Nurse Practitioner led approach, allowing the NP be the "captain" to take initiative and have ownership of the role, working independently and being the primary care provider to our patients. 50% of your time will be spent with patients, providing care. SUMMARY OF JOB RESPONSIBILITIES: The Hospice and Palliative Medicine Nurse Practitioner (HPMNP) is an advanced practice nurse, providing clinical care in accordance with state and federal practice law for patients in all hospice and palliative care settings. The HPCNP works in collaboration with the Hospice and Palliative Medicine Physician (HPMP) and the hospice interdisciplinary group (IDG) to ensure patients receive appropriate symptom control; facilitates discussions relative to the plan of care and advance directives; and provides critical regulatory and compliance requirements including face-to-face and billable/non-billable visits. The HPCNP collaborates externally with attending physicians, specialists, and other providers as applicable for the purpose of coordination of care to effectively meet the medical, psychological, and spiritual needs of the patient.
Requirements
- Board Certified Advanced Practice Nurse licensed in the state of Colorado.
- Controlled substance license with current DEA license number.
- Must have and maintain certification (or ACHPN) or obtain no later than the end of third year of employment.
- Current CPR Certification.
- Eligible or currently possess a Medicare/Medicaid UPIN number.
- Eligible or currently credentialed at local hospitals.
- Eligible to bill insurance for palliative care and symptom management visits.
- Valid Colorado driver’s license and unlimited access to reliable transportation.
- Adequate automobile insurance.
- Able to work in a variety of environments.
- Excellent interpersonal and communication skills.
- Compassionate communication with patients and their families, including discussing goals of care and end-of-life issues.
- Holds in confidence all patient records and information unless such information is properly released by the patient.
- Clinical excellence – skills and knowledge applicable to the patients served by HopeWest
- Knowledge of hospice regulations
- Knowledge of Board of Pharmacy rules around prescribing
Nice To Haves
- Prefer two years of experience in home care, palliative care, or oncology.
- Seeks or maintains certification in Palliative Medicine.

- Hiring salary range $112,800-124,800 - this is an exempt/salaried position
