HopeWest has a unique Nurse Practitioner led approach, allowing the NP be the "captain" to take initiative and have ownership of the role, working independently and being the primary care provider to our patients. 50% of your time will be spent with patients, providing care. SUMMARY OF JOB RESPONSIBILITIES: The Hospice and Palliative Medicine Nurse Practitioner (HPMNP) is an advanced practice nurse, providing clinical care in accordance with state and federal practice law for patients in all hospice and palliative care settings. The HPCNP works in collaboration with the Hospice and Palliative Medicine Physician (HPMP) and the hospice interdisciplinary group (IDG) to ensure patients receive appropriate symptom control; facilitates discussions relative to the plan of care and advance directives; and provides critical regulatory and compliance requirements including face-to-face and billable/non-billable visits. The HPCNP collaborates externally with attending physicians, specialists, and other providers as applicable for the purpose of coordination of care to effectively meet the medical, psychological, and spiritual needs of the patient.
