About The Position

The Hospice Admission Coordinator is responsible for coordinating the intake and admission process for new hospice patients, ensuring all necessary assessments, documentation, and legal requirements are completed in a timely and accurate manner. The coordinator works collaboratively with the admissions team, physicians, social workers, and chaplains to ensure a smooth and compassionate transition into hospice care.

Responsibilities

  • Coordinate and manage the intake process for patients being referred for hospice care.
  • Conduct initial assessments (clinical, physical, and psychosocial) for patients to determine eligibility for hospice care.
  • Ensure the completion of all necessary forms and documents, including consent forms and advanced directives.
  • Communicate effectively with patients and their families about the hospice program, explaining the services, benefits, and care options available.
  • Provide emotional support and educate patients and families about the hospice process, what to expect, and how to prepare for end-of-life care.
  • Ensure all required documentation for patient admission is complete and accurate, including clinical assessments, physician orders, and insurance documentation.
  • Maintain up-to-date knowledge of state and federal regulations regarding hospice care and ensure compliance with those regulations during the admission process.
  • Work closely with physicians, clinical staff, and social workers to develop a care plan tailored to the patient’s needs.
  • Serve as a liaison between the patient, family, and the interdisciplinary care team to ensure seamless coordination of care.
  • Provide a smooth transition from the hospital or other care settings to hospice care.
  • Coordinate the transfer of medical records, including test results and treatment plans, to the hospice care team.
  • Participate in quality assurance initiatives to evaluate and improve the admissions process and patient care services.
  • Ensure all admission processes are conducted in accordance with hospice policies and industry best practices.
