The coordinator works collaboratively with the admissions team, physicians, social workers, and chaplains to ensure a smooth and compassionate transition into hospice care.
Requirements
- At least 2 years of hospice experience

- Experience in patient admissions, case management, or healthcare coordination is a plus.
Responsibilities
- Patient Admissions: Coordinate and manage the intake process for patients being referred for hospice care.
- Conduct initial assessments (clinical, physical, and psychosocial) for patients to determine eligibility for hospice care.
- Ensure the completion of all necessary forms and documents, including consent forms and advanced directives.
- Patient and Family Communication: Communicate effectively with patients and their families about the hospice program, explaining the services, benefits, and care options available.
- Provide emotional support and educate patients and families about the hospice process, what to expect, and how to prepare for end-of-life care.
- Documentation and Compliance: Ensure all required documentation for patient admission is complete and accurate, including clinical assessments, physician orders, and insurance documentation.
- Maintain up-to-date knowledge of state and federal regulations regarding hospice care and ensure compliance with those regulations during the admission process.
- Collaboration with the Care Team: Work closely with physicians, clinical staff, and social workers to develop a care plan tailored to the patient’s needs.
- Serve as a liaison between the patient, family, and the interdisciplinary care team to ensure seamless coordination of care.
- Case Management: Provide a smooth transition from the hospital or other care settings to hospice care.
- Coordinate the transfer of medical records, including test results and treatment plans, to the hospice care team.
- Quality Assurance: Participate in quality assurance initiatives to evaluate and improve the admissions process and patient care services.
- Ensure all admission processes are conducted in accordance with hospice policies and industry best practices.
