The Admissions Registered Nurse (RN) plays a key role in welcoming patients and families to hospice care. This position focuses on assessing needs, developing care plans, and providing education and emotional support at the start of their hospice journey. You’ll have the opportunity to provide one-on-one care—both in person and over the phone—ensuring that every patient and family receives compassionate, individualized attention. Hours: 4 10 hour days with Tuesday as the day off. This is more than a nursing job—it’s a calling. Requirements
- Current New York State RN license
- Minimum 2 years of nursing experience preferred
- Strong assessment, communication, and critical thinking skills
- Compassionate and patient-centered approach to care
Nice To Haves
- Experience as a Hospice Nurse, Home Health RN, or in palliative care, oncology, or acute care strongly preferred
Responsibilities
- Conduct comprehensive patient assessments and initiate hospice services
- Develop and coordinate individualized care plans
- Provide education, counseling, and emotional support to patients and families
- Collaborate with the interdisciplinary team to ensure seamless transitions of care
- Cover patient care boards and make home visits as needed across our service area
- Participate in daily team reports and ongoing communication with staff
